I found it at Publix.
The products are sold at Walmart as well, but I didn't see the form there.
How to obtain your rebate:
Send your original cash register receipts dated between April 1 through Dec. 31 and 2 UPC symbols cut from the bottom o fhte box of formula.
INclude the completed form on the back of the slip.
Please allow 6 to8 weeks for delivery.
Requests must be postmarked by Jan. 31, 2012
