It's on page 14 of the insert.  You'll need to purchase three Johnson and Johnson Red Cross Brand , BAND-AID Brand Adhesive bandages, Cortaid, Neosporin OR BENADRYL topical products.

Then send in the certificate with a cash register receipt dated between April 1 through Sept. 30.

The kit will be delivered in 6 to 8 weeks.
