3) Submit cash register receipt dated between 10/4/11 and 12/31/11 imprinted with store name and purchase price
circled from Disney's The Lion King Diamond Edition Combo Pack Blu-ray™ or DVD (photocopies acceptable).
4) Submit cash register receipt dated between 9/26/11 and 12/31/11 with store name and item circled from
(1) participating Hefty® Foam, EZ Foil®, or Reynolds® product (see inset) (photocopies acceptable).
5) This original hand-printed, completed certificate.
